The sad demise of Bappi Lahiri, the singer-composer who wowed people with his disco music in the 80s and 90s due to Obstructive Sleep Apnea (OSA) has brought to light the need to understand what OSA is, people at risk and what the disease is all about.

In India, the incidence of OSA is about 3-17%, with males affected more than females among an age group of around 30 -50 years.

The most common risk factors include obesity, DM2, hypercholesterolemia, hypertension, depression. Luckily, we can treat OSA with surgical and non-surgical techniques which include weight reduction, reducing BP and anxiety and those who cannot be treated medically can be treated surgically.

OSA is an important health crisis that demands multi-layered intervention which if diagnosed early can be treated, and can improve the overall health of the individual.

What is OSA?

It is a common breathing problem. The breathing in such patients stops and starts repeatedly. The condition may cause a drop in the oxygen level in organs and cause abnormal heart rhythm.

The condition is caused due to too much relaxation of muscles at the back of the throat that narrows or completely blocks the airway for around 10 seconds.

It is common to hear that at least one member of the family snores when he/she sleeps.  The majority of the people ignore it. It’s important to realise that there are mainly two types of snoring – Primary snoring and snoring with sleep apnea.

Primary snoring consists of loud upper airway breathing sounds during sleep without episodes of apnea (cessation of breath) while snoring with sleep apnea is the loud sounds followed by episodes of stoppage of breathing.

People at risk

OSA is a global disease with a rising incidence associated with comorbidities. Obesity has a strong correlation with OSA, and in obese people due to increased fat deposition in the throat, the upper airway narrowing shall occur along with decreased muscle activity which leads to multiple hypoxic and apneic episodes through sleep.

The decreased oxygenation ultimately is a contributory factor to hypertension and cardiovascular diseases. These patients may have high blood pressure and alterations in cholesterol and blood sugar parameters.

Patients with severe OSA are at high risk for heart failure, coronary artery disease, and strokes. OSA patients with underlying heart disease may have sudden death due to multiple episodes of abnormal heart rhythm.

Symptoms to look for

It significantly affects the quality of life, leading to reduced focus, morning headache, insomnia, excessive daytime sleepiness, and irritability. Diagnosis of this condition is made through several tests, such as nocturnal polysomnography.

Treatment depends upon the severity of the condition and may include medication for treating underlying conditions, therapies, and surgeries. A healthy lifestyle is essential for managing OSA.


If not diagnosed and managed early, OSA may result in several complications, including dry eye, glaucoma, high blood pressure, diabetes, abnormal cholesterol levels, and gestational diabetes.

Studies have reported that patients with OSA are at a two-fold increased risk for sudden death than people who do not have a sleep disorder.


Some of the most common factors that could result in snoring with sleep apnea increase in the size of the tongue and surrounding areas of the throat, including the tonsils and adenoids and nasal deformities.


Once diagnosed, the doctor treats the patient according to the severity and underlying cause. The therapies for OSA include positive airway pressure machines and oral devices. Patients who are not benefitted from non-invasive treatment may be recommended surgeries to remove the obstruction.

Careful selection of patients is of paramount importance for surgical approaches. Mandibular advancement splint is offered in select patients as an alternative’

If diagnosed early, it can prevent cardiac disease, brain strokes, reduce memory disturbances and irritability and anger. In patients with obstructive sleep apnea, the gold standard of non-surgical treatment remains using a CPAP (continuous positive airway pressure) machine during sleep.

However, there have been advances in sleep surgeries in the last five years. Patient selection for surgeries would be based on their body mass index (BMI) levels and pre-operative sleep study also called pre-operative polysomnographic (PSG) results.

Some of the parameters that can be considered for surgery include basal metabolic rate of less than 40 and patients categorised under mild or moderate or early severe sleep apnea.

As a way forward, correction of sleep apnea through TORS procedure should also be part of the advice provided by ENT and physicians and cardiologist in improving the overall comorbidities management in the patient.

Correcting this ignored parameter will go the extra mile in remaining healthier throughout the day as well as maintaining a better quality of life.
